Description: Obesity is associated with significant comorbid conditions including type 2 diabetes, hypertension, cardiovascular disease, and cancer, as well as reduced quality of life and poorer mental health. While primary care providers often manage obesity-related comorbidities, obesity itself is not often directly addressed and remains undertreated. This educational program supports the consistent delivery of evidence-based obesity counseling and treatment in primary care settings. It focuses on improving the understanding and skills of primary care clinicians in working with their patients to manage their weight through evidence-based actions based in cultural humility.

Clinical Ambulatory Pharmacist - Population Health

I graduated from Wilkes University in 2015 with a Doctor of Pharmacy and BA - Spanish. Following graduation, I pursued PGY1 Pharmacy Residency at Penn State Health St. Joseph in Reading, PA and continued as staff pharmacist and ambulatory care preceptor for 3 years. I came to Jefferson Medical Group in April 2018. Under the direction of the population health team and physician network at Jefferson Health, our program was part of the Centers for Medicare and Medicaid's Comprehensive Primary Care Plus program. Between four pharmacists throughout Jefferson Health, we have about 60 primary care offices and over 300,000 patient lives, so we have focused on population health management to hone our efforts on patients we can make the most impact upon. for obesity management, I co-manage patients with obesity in primary care clinics under a CPA to help with safe titration of medications to goal. I also provide counseling and assistance in navigating cost issues. I have also implemented EHR improvements to help assist providers across a large enterprise health system.
